Purpose of the Post.
Ensure that the people of Gwynedd are at the heart of everything we do.
Lead a team of Community Care Workers (Domiciliary Care Service) on a local level to provide a Community Home Care Service that assists individuals to live their lives in the way they want to live it, in their own home.
Responsibility for functions  . e.g. staff, budgets, equipment
Staff – Management responsibility for the Community Care Workers (Domiciliary Care Service) on a local level.
Finance – Responsibility for monitoring expenditure in accordance with expectations on a local level (e.g., managing demand, and ensuring appropriate use of staff time).
Equipment – Responsibility for any personal equipment, and staff equipment, used to fulfil the role. Responsibility for assessing, and where appropriate, ordering suitable equipment in accordance with the requirements of the BTEC qualification provision of equipment.
Main Duties
Work with the individuals who receive a service, their carers, and other key workers involved in the care, to identify 'What matters to them' and find ways to meet their personal objectives.
Empower the voice of the individual. Supporting individuals to get advocacy support when needed, ensuring that the individual's needs and wishes are heard and addressed.
Supporting individuals to have an active part in making decisions about care and support in their life.
Work with individuals and their carers to plan a range of services to meet their personal objectives.
Assessing adults with less complex needs when appropriate, undertaking case coordinator responsibilities in accordance with the Social Services and Wellbeing Act 2014, and bringing in professional expertise as needed.
Responsibility for contributing to, and for developing a joint care plan with the individual and the CRT, considering the risks, when undertaking the case coordinator role.
Assisting individuals and their carers to meet personal objectives by providing information, advice, and support.
Research and ensure an understanding of community resources that exist within the area to be able to facilitate the well-being of the individuals. Responsibility for sharing this information and encouraging the Community Care Workers to make the best use of the resources available.
Establish and maintain positive connections in the community (e.g., third sector providers) and be able to refer individuals to the wider support services as needed.
Monitor the use and the arrangements in place in terms of getting specialist equipment for the home to promote opportunities that enables people to live as independently as possible in the community/in their homes in accordance with their wishes.
Work in partnership with co-workers in health and care, including social workers, community nurses, occupational therapists, physiotherapists, GPs, 3rd sector partners, and any other relevant workers.
Reporting on the individuals' personal well-being, and the ways they can be supported, in multi-disciplinary meetings, helping them to help themselves and using all the resources available in that area.
Identify needs that cannot be met, or obstacles to achieving what matters to individuals. Analyse and summarise the need, with responsibility for reporting and discussing possible solutions together with the CRT.
Supervise staff effectively in accordance with RISCA regulations, including monitoring the workload of staff members, and completing an annual evaluation in accordance with Council’s policy.
Ensure that the staff have the necessary skills to undertake the work safely and adequately, monitoring the training program, and a responsibility for developing the staff.
Ensure that the staff registration procedure is followed, in accordance with any Social Care Wales requirements, and that the staff are aware of the professional code.
Work with members of the CRT to carry out an ongoing assessment of demand and resources locally.
Depending on the demand, and the ability to prioritise locally, a joint responsibility with the Registered Manager for advertising, appointing, and inducting new staff in the area.
Ensure that all staff members have an appropriate DBS disclosure.
Responsibility for using the service’s system e.g., CACI, inputting information about service users, planning work for the Community Care Workers, and recording information correctly on the system.
Ensuring that the Community Care Workers make the correct use of the PDA. This may include training new staff on how to use them and continuously monitoring correct use.
Ensure that the work patterns are organized to make the best use of available staff time and resources locally, that meets the care needs of the individuals.
Being able to deal with issues that arise 'on call', using the appropriate skills and knowledge to do so.
Ensure suitable guidance and support for staff, when they contact in a crisis situation, by making use of any relevant guidelines.
Take responsibility for collecting and reporting management data on a local level e.g., reporting monthly on illness, recording the information electronically, and preparing reports in accordance with requirements.
Contribute to monitoring and reviewing the standard of the service, ensuring consistency in our arrangements and the provision of care for our residents.
Practice in a manner that is in accordance with the professional code of practice for social care.
Work as part of a team that supports colleagues at all levels and make connections locally and across the service.
Organize and lead staff meetings, and keep minutes in accordance with RISCA, with responsibility for reporting back on any issues that need to come to the attention of the Registered Manager.
Be prepared to contribute to developing policies and guidelines for the service.
Be part of the process to deal with complaints and compliments, and contribute to the investigation process.
Ensure that all staff absences are recorded in accordance with the service/Human Resources policies.
Responsibility for carrying out risk assessments that are relevant to the work, the staff, and the care provision of the individual.
Ensure compliance with any risk management arrangements identified in the risk assessments relevant to the work.
Take an active part in team meetings and any other relevant meetings as necessary to contribute to the development of the service.
Responsibility for safeguarding issues, considering risk, safeguarding and protecting adults. Dealing with any adult protection concerns promptly and appropriately by following the agreed procedure and notifying the relevant professional officers without delay.
OTHER RESPONSIBILITIES
Responsibility for self-development and undertaking any training that has been identified as relevant to the job e.g., a relevant Professional Qualification, BTEC Provision of Equipment.
Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
Operate within the Council’s policies in relation to equal opportunities and equality.
Responsible for managing information in accordance with the Council’s information management guidelines.
Ensure that personal information is treated in accordance with Data Protection legislation.
Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
Responsibility to report any concern or suspicion that a child or vulnerable adult is being abused.
Special Circumstances  . e.g. the need to work unsociable hours, special working arrangements etc.
Expectation to work beyond the work area boundaries occasionally.
At times, in special circumstances, it will be required to undertake the duties of a colleague to ensure continuity of service (e.g., due to illness or holidays).
Be prepared to undertake duties outside of service hours, by being part of an 'on call' rota.
In accordance with the demand within the service, it is accepted that, at times, it will be necessary for the individual to work flexible, giving more emphasis to some aspects of the job description than others e.g., a planning or supervisory role.
