Purpose of the post
•Ensure that the people of Gwynedd are at the heart of everything we do.
•To support the application, assessment, referral and monitoring processes and associated administration of the Council's Housing Support Grant across the County of Gwynedd; both accommodation based and floating support across the whole range of housing support needs.
Responsibility for functions
•Responsible for collecting data for the Housing Support Grant.
•Responsible for handling / analysing data relating to the Siop Un Stop / Housing Support Grant
Main duties
•To receive and evaluate all referrals for the Housing Support Grant funded housing-related floating support services in Gwynedd for the whole range of client groups i.e. homelessness, mental health, young people, care leavers, substance misuse, older people, learning disabilities etc.
•To undertake initial vetting and risk checks for all housing-related floating support referrals received by the Housing Support Grant team and where appropriate check and challenge information and methodologies used.
•To be the first point of contact for enquires from (from both professionals and members of the public) for the Council's housing-related support services.
•To match the referral to the most appropriate housing-related support.
•To work in partnership with the Council’s Supporting People funded housing-related support services, Housing Solutions service, Social Care service and partner organisations (such as Probation and Betsi Cadwaladr Health Board) to ensure the service user is able to access the most appropriate service to meet their identified needs.
•To be able to identify and understand the needs of multiple / complex service users to identify those agencies who are involved or have been involved
•To work with agencies, providers and service users to overcome barriers to achieve the best placement for service users.
•To disseminate information and encourage good practice with regards to clients who have multiple / complex needs.
•To arrange and attend multi agency meetings to achieve best placements for service users with multiple / complex needs.
•To respond to information received from the referrers to make decisions about individual support services.
•To ensure that all electronic information about service users seeking and receiving a housing-related support service is kept up to date working in line with data protection guidelines.
•Maintain and input into systems for referrals and allocations and manage gaps and waiting lists where necessary, using appropriate IT programmes to support this work.
•To work closely with the Housing Support Grant Team Leader and team to ensure operations run efficiently and effectively.
•To deliver the Council’s housing-related support services in accordance with Gwynedd’s Housing Support Grant Local Commissioning Strategy and the Welsh Government guidance and grant conditions.
•To be aware of and to take forward policy and guidance in relation to the Protection of Vulnerable Adults and children's safeguarding.
•To be aware of the support needs, characteristics and risks associated with each client group
•To establish and maintain effective communications and good working relationships with relevant statutory agencies, professional, independent and voluntary sector organisations, users, carers and other key stakeholders
•Collate and provide information and make recommendations in relation to the improvement of services in line with the Housing Support Grant Commissioning Process.
•Attend relevant multi-agency forums to provide housing advice and refer to other professionals.
•Responsibility for self-development.
•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
•Operate within the Council’s policies in relation to equal opportunities and equality.
•Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
•Responsibility to report any concern or suspicion that a child or vulnerable adult is being abused.
Special circumstances
•Occasionally you wil be required to work unsociable hours.
