ESSENTIAL
Good standard of education to level 5 GCSE grade C or above to include Welsh and English.
A suitable level 3 qualification in assessment or a higher/similar qualification in care. The current accepted qualification in accordance with the Social Care Qualifications Framework for Wales is the Open University HE Certificate in Social Care Practice (K101/102 & KZW113). You must therefore be willing and have the ability to fulfill the qualification as a condition of accepting the job unless you already possess it (or a higher qualification).DESIRABLE

- Experience of working with children and families who show a range of complex needs (mental health challenges, behavioural difficulties, drug and alcohol abuse, domestic violence) and demonstrate an ability to create rapport and build effective relationships.

Job Description
Purpose of the Post.
• Ensure that the people of Gwynedd are at the heart of everything we do.
• The team works in circumstances to: Preventing a child from going into care unless absolutely necessary. Returning a child home within the first 8 weeks of going into care. Return a child home if it is safe to do so.
• Supporting individuals to move between a range of settings (e.g. from residential to fostering).
• By providing intensive intervention for a period of time, the team works with families through periods of crises in order to stabilize the situation and reduce risk. Research has shown that the relationship with the family is the most influential factor to contribute to success. The employee needs to be honest, reliable and open. Also, the employee needs to persevere despite any obstacles that may arise. For these reasons, individual caseload is kept low in order to provide intensive intervention, build a positive relationship and ensure effective support.Responsibility for functions . e.g. staff, budgets, equipment

General Duties
• Practice within the relevant legislative frameworks and guidance that underpin the provision of social services in Wales
• Apply the code of professional practice
• Maintain up to date knowledge and maintain own professional development
• Actively participate in regular reflective supervision by registered social worker or registered occupational therapist (dependant on role and tasks ) and seek advice and support on areas of work beyond expected parameters of work
• Manage and be accountable for your own work and work within the parameters of the role
• Work collaboratively with multi-disciplinary colleagues under guidance from your line manager/registered social worker
• Prepare and present reports and records under the supervision of line manager/ registered social worker ensuring information is correct, consistent and shared safely and appropriately
• Support and empower people to participate in decision making
• Empower people to have a voice and support them to access advocacy services
• Work collaboratively with people in need of care and support and other professionals
• Contribute to strength-based assessments and practice in a way that promotes self-determination, independence and empowerment under the supervision of a registered social worker .
• Contribute to outcome focussed assessment using a ‘What Matters ‘conversation to identify what is important to that individual, eligible needs and to develop outcome focussed plans that facilitate care and support with guidance from you line manager and other more experienced colleagues.

• Identify an Adult or Child at risk in accordance with relevant legislation and the Wales Safeguarding Procedures and report appropriately.
• Undertake the relevant level 4 qualification as set out in the SCW Qualification Framework and other level 3 and 4 programmes as required.

• Design an intervention package based on a range of research-based interventions. It will be necessary to use discretion and creativity to adapt packages according to the specific needs of the individuals. There will not necessarily be an intervention package that is fit for purpose without interweaving with various other interventions due to the complexity of cases.
• Lead on an intervention plan that is tailored to the needs of children and their carers.
• Conduct reviews of progress, strengths, and risk with your families regularly, including social workers and other agencies as part of the review
• Making decisions based on evidence.
• Agree with parents and families and other agencies on the aims and purpose of the work within safeguarding policies and procedures
• Record information and prepare reports in accordance with the requirements of the system. To participate in court proceedings including giving evidence and preparing reports.
• Promote the empowerment of children's voice and support it to get advocacy support.
• Working in partnership with other professions and agencies to promote the family's objectives, putting the children at the centre.
• Find solutions to overcome obstacles. Reporting obstacles that cannot be overcome locally to the leader.
• Provide information and advice to children and their carers about the support available from the service and also refer them (with permission) to the wider support services available in the community from local and national groups and organisations.
