Purpose of the post
•Ensure that the people of Gwynedd are at the heart of everything we do.
•Assist individuals with learning disabilities to live their lives in the way they wish to live, assist them to develop skills and ensure opportunities to be a part of their community.
Responsibility for functions
•Ensure appropriate use of specialist equipment including reporting on deficiencies and following practice protocols and specialist training as required.
•Ensure appropriate use of users' property e.g. medicine and cash.
Main duties
MAIN DUTIES AND KEY TASKS:
SUPPORT THE INDIVIDUALS WHO USE THE SERVICE
•Work to meet the well-being objectives of the individuals you are supporting and ensure and respect individuals' rights and empower their voice.
•Follow the Active Support ethos (for more information click here: Active support <https://www.bild.org.uk/wp-content/uploads/2020/06/What-is-Active-Support.pdf>) and Positive Behaviour Support (Click here for more information: Positive Behaviour Support <https://www.youtube.com/watch?v=epjud2Of610>), that emphasise the importance of working with individuals and promote their independence.
•Assist with personal care in a dignified and respected manner.
•Contribute to and identify opportunities to amend and follow the individuals Personal Care Plans and encourage taking positive and well-informed risks.
•Contribute to the assessing, monitoring, planning and implementing process according to the needs of the individual.
•Test new ways of supporting and use creative solutions that draw on strengths.
COMMUNICATION AND RELATIONSHIPS
•Co-operate and communicate with professional workers and families/carers and search for solutions that can help. Contribute to multi-disciplinary discussions.
GUIDANCE AND POLICIES

TRAINING AND SUPERVISION
•Take advantage of the opportunity to receive supervision and mentoring from experienced staff members.
•Evaluate and follow Cyngor Gwynedd’s Learning and Development programmes that are mandatory for professional development.
•Commit to completing professional qualifications when needed.
SAFEGUARDING
•Follow local and national policies and procedures to identify, record, report and take part in POVA protocols.
•Mentor new staff in their development.
•Qualified to assess and order minor equipment for individuals.
•Responsible for administering, distributing and giving medication in accordance with policy.
•The ability to carry out simple health tasks.
•Carry out detailed assessments and identify changes in health and well-being.
•Respond appropriately and swiftly to emergency situations.
•Responsibility for self-development.
•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
•Operate within the Council’s policies in relation to equal opportunities and equality.
•Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
•Responsibility to report any concern or suspicion that a child or vulnerable adult is being abused.
Special circumstances
•The post-holder must be able to undertake manual handling tasks which can include wheelchairs and hoists.
•This job description is only a general guide. All support workers may be required to undertake duties that have not been outlined above at the Team Manager or the line manager's request.
•Expected to work across other service sites if necessary.
