Job Description
Purpose of Post
• Ensure that the people of Gwynedd are at the centre of everything we do.
• Plan, coordinate, and provide a programme of activities and projects for young people
• Build extensive networks to develop this work as much as possible.
• Collaborate with the rest of the Youth Service Team to ensure a smooth transition across the teams and that Health and Well-being is a key part of the service.
• Work with young people within the headings of the Five Ways to Well-being, and ensure that the voice of young people is in everything that is undertaken
• Develop an emotional resilience pathway that is non-clinical jointly with other Agencies.
• Coordinate the Mental Health group
• Implement a programme of activities and projects, ensuring the best possible experience for young people
• This post will focus on the 11-25 age groupResponsibility for Resources. . e.g. staff, finance, equipment
• Responsibility for oversight and organisation of the projects.
• Access to budget for specific projects / resources.
• Personal Equipment (ICT equipment, mobile phone etc).
• Responsible for ensuring that his / her own travel expenses are valid and appropriate.Main Duties
1. Support the Youth Engagement and Progression Coordinator in identifying and assessing indicators of homelessness and developing an early identification tool that can identify young people at risk of becoming homeless.
2. Support the Youth Engagement and Progression Coordinator to monitor and assess the relationships between young people most at risk of becoming homeless and young people most at risk of not engaging in education, training or work.
3. Support the Youth Engagement and Progression Coordinator to work with partners to ensure that the number of young people becoming homeless is known, and work towards a reduction in youth homelessness.
4. Work with key partners and services, including Housing, Health, Education, Social Care, Youth Justice and the Third Sector, to develop a collaborative and coordinated approach to ensuring that those young people at risk of homelessness receive help them get the support and accommodation that makes a difference to them.
5. Engage and support young people who have been identified as' at risk of homelessness OR work with 11-25 year olds, especially young people at risk of becoming NEET, those who are NEET and in care / after care care and interventions aimed at identifying individual barriers and co-ordinating a package of support to reduce the risk of homelessness.
6. Act as an advocate for the young people, representing their views and rights where appropriate
7. Work with key partners to analyse
accommodation and support across the Authority so that there is a clear picture of how well the provision is meeting the needs of the County's young people.
8. Maintain and develop a multi-agency panel (Youth Homelessness Forum) to identify, track and provide appropriate support to young people who have been identified as being at risk of becoming homeless or who are currently homeless. This will include developing policies, protocols and procedures to support the work of the panel.
9. Deliver Training to relevant partners so that they can develop awareness of youth homelessness and understand how they can effectively support young people.
10. To work with all relevant internal and external partners to raise awareness of homelessness in schools / post-16 education institutions.
11. Prepare, organize, plan, evaluate, and manage resources for each project / program.
12. Provide the key brokerage and advocate role for identified young people.
13. To work closely with partners / organizations and signpost young people to additional personal support as required.
14. Liaise with parents / guardians of young people to enable them to offer informed support
15. Prepare and support young people during key transition periods
16. Support the 16-25 Support Team Leader / Homelessness Team Leader in establishing appropriate performance measures to enable the Youth Service to measure the impact of the support provided to young people
17. Prepare progress reports including monitoring and tracking of young people's progress and outcomes.
18. To ensure a clear and accurate record of work is fed into an information management system, in line with the Youth Service's recording and monitoring systems.
19. To ensure the effective protection and handling of all personal data, in accordance with an agreed Youth Engagement and Progression Framework Protocol.
20. Attend and contribute to Youth Service planning meetings, Team meetings, Post 16 Engagement Panel / case conferences / Welsh Government meetings / other meetings as required.
21. Comply with any Youth Service procedures and systems.
22. Implement and adhere to safeguarding procedures when working with young people.
23. Adhere to the Occupational Standards for Youth Work and the Rights of the Child.
24. Ensure that a young person's Voice is heard through everything that is offered and developed
25. Attend youth worker training organized by or on behalf of the Council
26. To undertake other tasks and responsibilities of a similar nature as may be required from time to time by a Youth Support Team Leader aged 16-25 and a Homelessness Team Leader in relation to the smooth running of the service.
27. Responsible for self-development.
28. To ensure compliance with workplace Health and Safety rules in accordance with the responsibilities set out in the Health and Safety at Work Act 1974 and the Council's Health and Safety Policy.
29. To act within the Council's policies and procedures in relation to equal opportunities and equality.
30. Responsibility for information management in accordance with the Council's information management guidelines. Ensure personal information is handled in a manner that complies with Data Protection legislation.
31. Commitment to reducing the Council's carbon emissions in line with the Carbon Management Plan and encouraging others to work positively towards reducing the Council's carbon footprint.
32. Undertake any other reasonable duties commensurate with the salary and responsibility levels of the post.
