Job Description
Purpose of the post
•The purpose of the Integrated Adults Team is: “to enable me to live my life as I wish”.
•Ensure that the people of Gwynedd are at the centre of everything we do.
•Assist vulnerable individuals to live their lives in the way they wish to live.
•Undertake an advisory role on Moving with Dignity issues.
•Promote the Moving with Dignity strategy and policy within the service and to external services, individuals and their families.
•Follow the Moving with Dignity strategy and policy, carrying out complex manual handling assessments and reviews, facilitate early discharge from hospital, look into appropriate techniques and equipment to promote moving with dignity (aiming for single care), work with Occupational Therapists in Social Services and Betsi Cadwaladr Health Board, and provide relevant training for individuals, their families and care staff.
•Assess the needs of adults and their carers, in accordance with the requirements of the Social Services and Well-being (Wales) Act 2014 and other relevant legislation and guidance.
•Provide an Information, Advice and Support service to individuals to enable them to meet their personal goals.
•Assist people to identify solutions, prevent dependency and promote independence.
•Provide information about the support resources available in the community, from the third sector, the Council and the Health Board, to enable individuals to live as independently as possible and to maximise their quality of life.
•Ensure that individuals' needs are met in accordance with the Council's safeguarding arrangements and statutory responsibilities.
Responsibility for functions
•Staff - No direct management responsibilities but expectation to work as part of a team by providing training on moving with dignity, and supporting and mentoring colleagues.
•Budget - Provide practical and appropriate advice and guidance to service users to facilitate effective financial management in the Service.
•Equipment - Responsibility for all specialist equipment used to carry out the post. Demonstrate the best use of resources in regard to the provision of suitable adaptations and equipment, and provide a service in accordance with policies, procedures and guidelines. Responsibility for the keys to the storeroom where equipment is stored.
Main duties
•Work as part of an integrated multi-disciplinary team and respect the skills, expertise and contributions of colleagues within the team. Take joint responsibility for ensuring an integrated approach.
•Work with every Department within the Council and other partners who have a contribution to make to ensure that individuals meet their well-being goals.
•Represent the local authority in relevant meetings.

•Carry out and record comprehensive manual handling risk assessments, along with any other specialist assessment involving manual handling in accordance with the Moving with Dignity strategy and policy, guidance and legislation, to manage risk for the individual and carers.
•Provide and demonstrate the use of specialist techniques or equipment to individuals, carers/families.
•Work with reps to increase specialist knowledge and access specialist equipment for the individuals.
•As part of a manual handling assessment and referral for minor & major adaptations and complex structural adaptations (including extensions). Consider the need for appropriate accommodation. Ensure that the adaptation is completed to the expected standard which meets the need.
•Refer on to the third sector, other professions (e.g. Physiotherapist) when required.
•Provide Moving with Dignity training to Occupational Therapists and carers.
•Ensure compliance with any risk managing arrangements identified in risk assessments that are relevant to the work.
•Maintain and record complex and comprehensive manual handling plans in accordance with guidelines and legislations, in order to ensure that the care provision meets the personal goals of the individual.
•Exercise professional judgement and decision-making in complex situations.
•Advise colleagues on complex areas of manual handling.
•Prepare and submit professional reports and records, ensuring that the information is correct, consistent and is shared securely and appropriately.
•Facilitate hospital discharges for individuals with complex manual handling needs by working with Betsi Cadwaladr Health Board professionals.
•Retain ownership of cases. Undertake responsibilities of a care coordinator by 'bringing in' expertise as and when needed.
•Ensure that equipment is returned when no longer needed.
•Implement the professional code of practice as a registered professional with the Health and Care Professions Council (HCPC).
•Possess the latest information and maintain your own professional development and keep a portfolio of continuous professional development evidence. Develop practice through supervision and reflection.
•Actively participate in regular personal and group supervision sessions and in the annual appraisal process.
•Manage and be accountable for your own work.
•Record accurately, commensurately and timely to ensure that our integrated recording systems are up to date.
•Be accountable for the quality of the work and take responsibility for maintaining and developing knowledge and skills.
•Practise within the legislative frameworks and relevant guidelines that underpin the social services provision in Wales.
•Assess individuals' mental capacity in accordance with the requirements of the Mental Capacity Act.
•Be aware of the steps that should be taken in cases where there are allegations of harm or abuse in accordance with section 7 of the Social Services and Well-being Act. Complete investigations into allegations of abuse.
•Ensure compliance with Health and Safety in the Workplace rules in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety policy.
•Commitment to providing high-level customer care and assisting people in accordance with the principles of the Social Services and Well-being Act 2014.
•Operate within the Council’s policies and procedures in relation to equal opportunity, equality and sustainability.
•Be responsible for managing information in accordance with the Council’s information management guidelines and standards. Ensure that personal information is handled in a way that complies with Data Protection legislation.
•A commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s carbon footprint.
•To undertake any other reasonable duty that corresponds to the salary level and responsibility level of the post.

Special circumstances
•Working during unsociable hours as and when needed.
•Consideration will be given to extending the service's opening hours during the week, along with working weekends.
