Purpose of the Post.
Working as part of a residential team:
• Ensure that the people of Gwynedd are at the centre of everything we do.
• As a team member, provide direct care and preventative interventions for children and young people on a day-to-day basis along with other support duties and tasks within the unit.
• Work closely with internal and external partners to achieve the best outcomes for service users.
• Provide a range of interventions using the most effective and appropriate intervention to correspond with the needs of children and their families.
• This work is part of a wider project therefore, as required, you will need to work in other residential homes in Gwynedd.
Responsibility for functions . e.g. staff, budgets, equipment
• Equipment – kitchen and catering, ensuring appropriate use of the building's facilities

3 Main Duties
• Contribute to drawing up care and support plans, participate in reviews and other meetings, and help in implementing the plans.
• Be a team member, supporting colleagues and receiving support as needed.
• Develop a good working relationship with the children to develop trust, improve contact and access to services and assist with communication and coordination.
• Develop the resilience and confidence of families to empower them to make informed decisions and take responsibility for their own lives by providing them with the necessary skills.
• Attend Planning and Review Meetings and contribute to the development of an action plan. Keep notes of all meetings attended and the case work undertaken. Provide a summary and reports on case work.
• Encourage and support to follow the action plan.
• Draw up and provide a range of interventions to support children in their local communities.
• Support and enable Children to live independently within the community.
• Support and enable Children to reach their full potential, in a manner that promotes their well-being, health, development and safety.
• Ensure that the service responds to the child's needs by following a behaviour plan as required.
• Assist and support to care for the children.
• Be vigilant of signs of unhappiness and abuse and ensure that children are protected and monitored.
• Inform the line manager or any other suitable person of any poor or concerning practice or any evidence that mentions this.
• To take care of young people's physical needs as required, e.g., cooking, laundry, ironing, shopping, washing, managing money or by assisting the children to complete these tasks.
• Inform colleagues of any relevant developments/incidents e.g., when changing shifts.
• To speak to children and listen to them, recording anything significant.
• Attend team meetings and any other meetings as required and considered suitable by the Team Manager.
• Receive regular supervision.
• Be flexible, within reasonable boundaries, to ensure sufficient staff provision for the unit
• Responsibility for self-development.
• Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
• Operate within the Council’s policies in relation to equal opportunities and equality.
• Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
• Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
• Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
• Responsibility to report any concern or suspicion that a child or vulnerable adult is being abused.
Special Circumstances . e.g. the need to work unsociable hours, special working arrangements etc.
• The nature of the job means working outside normal office hours, including weekends.
• The post is located at Deiniolen but as required, to work in other residential homes in Gwynedd
