Job Description
Purpose of the post
The purpose of the Integrated Adults Team is:
"to enable me to live my life as I wish"
•Ensure that the people of Gwynedd are at the heart of everything we do.
•Give operational leadership and lead change across a workforce group employed by Gwynedd Council and Betsi Cadwaladr Health Board to ensure that people in the area are able to gain access to the right person at the right time and receive the right support to enable them to continue living their own lives.
•Support the Area Leader to lead and manage services for older people, adults with a physical and sensory disability and adults with memory problems and their carers.
•Provide expertise in specific areas and operate in a consultancy role for the Service on a County wide level.
Responsibility for functions
•STAFF - Day to day supervision responsibility for all the officers working within the team. Professional responsibility for all the officers from the same profession working within the team and within other area teams.
•FINANCE - support the Area Manager to monitor and manage the budget for the area.
•EQUIPMENT– support the area manager to take responsibility for all specialist equipment used by the team's staff.
Main duties
MANAGEMENT RESPONSIBILITIES
•Lead and Manage People – motivate, encourage and empower staff by delegating work effectively and creating an environment of respect, collaboration and risk-taking. Give direction and feedback to staff by transferring the vision effectively by focusing on providing a service.
•Managing Resources - plan and monitor to secure a high quality service and continuous improvement. Provide value for money by purchasing, prioritising, managing, and monitoring effective use of resources and acknowledging that knowledge, ability and skills are also resources.
•Performance and Development – Identify staff strengths and areas for development via regular supervision and appraisal. Focus on improving performance through effective management and development and by measuring, monitoring and evaluating performance. Implement and develop business plans to reflect this.
•Solving problems – make things happen by taking meaningful decisions and responding effectively to priorities as they change.
•Communication – share and listen to information, views and ideas without preconception by using appropriate communication methods. Increase motivation by using inclusive communication that acknowledges other people’s feelings.
•Awareness of the context – effective working and understanding within the Council’s political framework and to be aware of the issues that have an impact on service provision at different levels. Nurture links with others to benefit from best practice in order to improve the provision for Service Users.

SPECIFIC RESPONSIBILITIES
•Work with citizens who require support to enable them to meet the outcomes that are important to them.
•Act in accordance with the requirements of the Social Services and Well-being Act 2014 and other relevant legislation and guidelines.
•Encourage the implementation of preventative models to reduce the dependency on services.
•Ensure a safe environment for team members to trial new ways of working. Empower staff to take dependable professional decisions.
•Report on barriers that the team cannot overcome to the Area Leader.
•Provide line manager accountability for team members. Take a lead role for the day to day issues for the area (leave, sickness absence matters etc.).
•To be a key professional person for the integrated team, and provide professional support and supervision to the Social Work service/ Occupational Therapy / social work practitioners (depending on the profession) within the work area and across the County.
•Ensure that members of the team receive sufficient and regular supervision.
•Provide professional support and advice to the integrated team members that will inform good practice and allow for an effective working relationship.
•Encourage workforce development and encourage the support of students on placement.
•Responsibility for appraising staff and supporting them to create personal development plans and practice improvement plans.
•Put a system in place to deal with poor performance and be confident to implement capabilities and discipline steps where needed and deal with these in the initial steps.
•Confident to manage daily decisions in all aspects of the budget and support the area leader to scrutinise and validate the budget regularly. Ensure effective budgetary control within the team.
•Support the Area Manager to ensure that all team members explain the Council’s Charging Policies for services to every individual. Lead on developing close a relationship with the Income and Welfare Department to ensure that every service user receives support to apply for financial support from the Council to pay for services.
•Support the Area Manager in ensuring that a standard contract has been completed between the individual and the Council for all services provided.
•Support the Area Leader to deal with enquiries and investigate and complete reports to respond to complaints.
•Ensure that the legal responsibilities and departmental policies are met in relation to the team's work.
•Ensure that individuals' needs are met in accordance with the Council's Safeguarding arrangements and statutory responsibilities.
•Act as designated leader on safeguarding cases. Complete investigations into allegations of abuse.
•Encourage and facilitate regular multi-disciplinary discussions to discover creative ways of fulfilling the outcomes of the citizen without restricting the intervention to match people and traditional services and create dependency on services.
•Distribute work and ensure a suitable response. Analyse the urgent nature/level and implications of any risks noted and respond suitably and in a timely manner.
•Collaborate as part of a multi-disciplinary team and respect the skills, expertise and contributions of colleagues within the team.
•Collaborate to create strong links with other Council departments and other partners which have a contribution to make to ensure that citizens meet their well-being outcomes.
•Contribute to fulfilling the Council's well-being outcomes and provide information and map the community services provision which contributes to fulfilling the citizen's care needs.
•Contribute and lead project teams locally and on a County level.
•Contribute to developing and supporting the Information, Advice and Support service for the area and on a County level.
•Represent the department in local, County-based and regional meetings.
•Contribute to shaping the commissioning procedure for adults social care. Contribute information and ideas to the development of a commissioning strategy.
•Contribute to the Population Needs Assessment.
•Develop expertise in specific fields and act as an expert to provide specific advice and guidance to the service locally and on a County level.
•Delegate on behalf of the Area Leader.
•Support the Safeguarding Team to ensure that Best Interest DOLS assessments are completed in accordance with legislation.
OTHER RESPONSIBILITIES
•Responsibility for self-development.
•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.

Special circumstances
Work unsociable hours according to the need.
There is consideration to extend the opening hours of the service during the week and weekend working.
The post holder will be expected to be part of any arrangements that will be established on a rota basis.
