Purpose of the post
•Ensure that the people of Gwynedd are at the heart of everything we do.
•Assist vulnerable individuals to live their lives in the way they wish to live.
•Ensure that individuals' needs are met in accordance with the Council's safeguarding arrangements and statutory responsibilities. There will also be an expectation to comply with all the requirements of the Care Act 2016

•Responsibility for ensuring that the resources, tools, and equipment used are in a good condition and are fit for purpose.

Main duties
SUPPORT THE INDIVIDUALS WHO USE THE SERVICE
•Support to Lead and implement the provision of person-centred services.
•Support individuals to take an active part in making decisions about care and support in their lives.
•Collaborate with citizens and their carers to identify ‘What matters to them’ and discover ways of meeting personal objectives.
•Empower the individual’s voice. Support individuals to get advocacy support.
•Receive referrals for services for older people following a professional assessment, ensure correct assessment prior to access with evidence of the ability to address the needs
•Ensure that the person is central and is part of life decisions. And that they live their lives as they wish.
•Draw-up unique and specific programmes that meet with the individual's needs and review and monitor these regularly.
•Assist people to identify solutions, to prevent dependency and promote independence.
•Implement the relevant legislation (e.g., Health and Safety, the Well-being of Future Generations Act, Social Services and Well-being Act) and ensure that they adhere to them.
•Have an overview, understanding and effective Management of the service requirements and be willing to respond in a timely manner to comply with the requirements.
•Ensure that care and support is of the best standard and meet with all the requirements of the Care and Well-being Act 2014.
•It will be necessary to register with CCW
•Undertake inspections and collaborate with CIW and respond and act on what comes to light
•Lead in their role and collaborate closely with the manager to take ownership of the principles of Ffordd Gwynedd and promote and encourage staff well-being.
•Supervise, lead, motivate and advise staff to work as a team and to follow Council guidelines and policies, to ensure the best for the people of Gwynedd.
•Contribute towards evaluating and reviewing the work standards of the team and service in general.
•Ensure that the workforce meets with the training requirements in accordance with the national standards
•Support disciplinary issues in consultation with the personnel officer.
•Be part of the process of recruiting and appointing suitable staff to the service, and ensure appropriate DBS disclosures
•Undertaking the process of the induction of new staff
•Give support to ensure that the supervision and development of staff exists in accordance with the Council’s policies and guidelines and that every member of staff is appraised annually.
•Support and collaborate Ensure that the procedure for managing absences is followed,
•Collaborate to ensure that employees use the Department's documentation and recording methods.
•Ensure that the staff registration system is followed, in accordance with the requirements of Social Care Wales. Also, that staff are aware of the professional code.
•Deal with complaints and compliments, assisting with the investigation process.
•Promote Well-being and self-control
•Ensure compliance with any risk assessment arrangements identified in risk assessments that are relevant to the work.
•Collaborate and support with ordering, receiving records, distribution, and safe disposal of medication.
•Measure staff competency by ensuring that they are qualified to distribute medicine
•Collaboration to ensure that the quality of the service meets with the standards set
•Communicate openly with agencies, families, and professionals
•Collaborate with drawing-up reports, following Quality Assurance monitoring of the service and be accountable to the responsible person
•Collaborate on developing policies and procedures and review relevant service policies.
•Liaise with other agencies on behalf of the Department. Promote partnership working with Service Users and other agencies e.g., health authority and the third sector.
•Transfer professional and departmental information and knowledge.
•Share professional opinion on cases with others within the service as required.
•Notify the Manager of cases and situations that raise key policy questions or cause serious problems for the Department so that specialist guidance could be received or develop a new specific policy.
•Attend and contribute to county Team Managers meetings in the manager's absence
•Contribute towards in-service training and to training activities as appropriate.

•Ensure compliance with Health and Safety regulations in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.

•Be responsible for managing information in accordance with the Council’s information management standards and guidelines. Ensuring that personal information is treated in accordance with Data Protection legislation.
•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
•Responsible for reporting any concern or suspicion that a child or vulnerable adult is being abused.
Special circumstances
•The post may mean working unsociable hours outside normal office hours on occasions.
•A DBS will need to be completed, and to register with Care Council Wales
•To be ready to support in other services when asked
