Job Description
Purpose of the post
•Ensure that the people of Gwynedd are at the centre of everything we do.
•Give operational leadership and lead/manage change for a workforce employed by Gwynedd Council and collaborate with the Betsi Cadwaladr Health Board workforce with the aim of working in an integrated manner to ensure that people in the area are able to gain access to the right person at the right time and receive the right support to enable them to continue living their own lives.
•Lead and manage services for adults with a physical and sensory disability and adults with memory problems and their carers.

Responsibility for functions
•STAFF - Professional responsibility for all the officers working within the team.
•FINANCE - monitor and control the budget for the area.
•EQUIPMENT– Responsibility for all specialist equipment used by the team's staff.
Main duties
MANAGEMENT RESPONSIBILITIES
•Remove obstacles to allow for an effective service provision. Make things happen by taking meaningful decisions and by responding effectively to priorities as they change.
•Lead and Manage People - Motivate, encourage and empower staff by delegating work effectively and create an environment of respect, collaboration and risk-taking. Give direction and feedback to staff by transferring the vision effectively by focusing on providing a service.
•Managing Resources - Plan and monitor to secure a high quality service and continuous improvement. Provide value for money by purchasing, prioritising, managing and monitoring effective use of resources and recognising that knowledge, ability and skills are also resources. Be responsible for the day to day management of relevant budgets, ensuring that expenditure does not exceed the allocation.
•Performance and Development – Identify staff strengths and areas for development via regular supervision and appraisal. Focus on improving performance through effective management and development and by measuring, monitoring and evaluating performance. Implement and develop business plans that reflect this.
•Communication - Share and listen to information, views and ideas without preconception by using appropriate communication methods. SPECIFIC RESPONSIBILITIES
•Lead locally to provide services that citizens need in a sustainable way and of a high quality.
•Lead role in collaboration with Betsi Cadwaladr Health Board and the third sector to establish and develop an integrated care service.
•Promote effective integration within the Area and across Gwynedd. Nurture firm connections with agencies such as the third sector in order to provide efficient integrated care.
•Convey a sense of working towards the same objective.
•Drive a culture of change, innovation and modernisation to facilitate service improvements.
•Support the team to map work-flow processes. Re-plan services and implement changes.
•Collaborate closely with the other leaders in the team to eliminate obstacles. Report on obstacles in weekly leaders' meetings.
•Lead on ensuring that the team implements the principles of the Social Services and Well-being Act 2014.
•Ensure that the 'What matters to me' and determining personal outcomes conversations take place and are implemented across the service.

•Lead on safeguarding matters within the area in accordance with statutory requirements and the Council's policies and procedures.

•Supervision responsibility for the senior practitioners and members of the team.
•Deal with complaints and enquiries with the support of the Customer Care Officer.
•Lead on Human Resources matters within the social services workforce in the integrated team. Responsibility for recruiting and managing staff including evaluation, development plans, performance management, sickness absence and matters involving complaints and discipline.
•Take responsibility for Continuing Health Care disputes at level 1 under the North Wales Continuing Health Care arrangements.
•Lead and manage specific work programmes based on the Area and whole County.
•Provide an overview of the Information, Advice and Support service.
•Represent the Department in local, regional and national meetings as required.
•Lead on developing measures that measure success. Analyse and evaluate the information to ensure that the interventions meet the personal objectives of citizens and that the resources of the integrated team are used efficiently and effectively.
•Report on measures in departmental performance meetings.
•Lead on ensuring that the area takes ownership of efficiency savings schemes.
•Contribute to shaping the commissioning procedure for adults social care. Contribute information and ideas to the development of a commissioning strategy.
•Ensure that team members explain the Council's charging policy for services to all service users. Lead on developing a close relationship with the the Income and Welfare Unit to ensure that every service user receives support to apply for financial support from the Council to pay for their services.
•Lead on ensuring that a standard agreement is completed between the Council and users for every service provided.
•Support opportunities to introduce new technology to transform working practices.
•Promote the commitment of the third sector to work in an integrated manner in the area.
•Delegate on behalf of the Senior Adults Manager.
•Support the Safeguarding Department to ensure that DoLS Best Interest assessments are completed in accordance with the legal requirements.
OTHER RESPONSIBILITIES
•Responsibility for self-development.
•Ensure compliance with workplace Health and Safety regulations in accordance with the responsibilities noted in the Health and Safety at Work etc. Act 1974 and the Council’s Health and Safety Policy.

Special circumstances
•Working unsociable hours occasionally as and when necessary.
•Consideration is being given to extending the service's opening hours during the week along with working weekends.
