Job Description
Purpose of the post
•Assess the needs of disabled children and children who are ill and their families/carers in accordance with the relevant statutory requirements
•Work as a member of the county multi-disciplinary team.
•Carry out joint duties such as establishing care plans, monitor and review these to respond to the needs.
•Assist to develop new services for disabled children and children who are ill and their families within the County.
•Ensure that the people of Gwynedd are at the centre of everything we do.

Main duties
•Be a member of the social work team within the multi-agency service of the Children's Specialist Service.
•Undertake relevant duties in accordance with the service guidelines and policies.
•Participate in any task groups that are relevant to the post.
•Promote the aims of the service by working closely with fellow members of each agency.
•Be a contact worker or/and a key worker to many disabled children and children who are ill following the service referral procedure.
•Assess the needs of these children in accordance with the relevant statutory requirements.
•Assess the needs of carers / families in accordance with statutory requirements.
•Undertake social work tasks and duties with the children and their families.
•Organise and provide care packages to meet with the needs assessed in consultation with the children, their families and co-workers.
•Conduct relevant risk assessments for the above.
•Monitor and review care plans provided via the multi-agency system.
•Be part of the social work team's duty system.
•Promote links with service providers and relevant Voluntary Organisations.
•Work jointly with groups / parents' forums
•Work jointly with the Child Protection Team and the Looked After Children Team in relation to child protection cases and with children who receive accommodation.
•Work jointly with relevant adult teams in terms of planning for transition age.
•Provide information and advice to co-workers as appropriate
•Follow training that is relevant to the post and the Department's Staff Development Plan.
•Follow the Staff Supervision procedure.

•Any other relevant duties according to the requirements of the Director.
•Prepare relevant documents in response to the assessment process and provide social care plans, assessments and any work recording and keeping statistics that are relevant to the post.
•Be aware of the relevant statutory duties for disabled children and children who are ill.
•Be aware of statutory health and safety duties and relevant department guidance and policies.
•Follow training that is relevant to the post and Plan.
•Develop the Department's staff.
•Follow the staff supervision procedure.
•Be aware of relevant policies and the organisation of the departments for disabled children and children who are ill.
•Develop specialist knowledge and skills in the area of disabled children and children who are ill.
•Undertake duties that are relevant to the Gwynedd Register of Disabled Children.
•Input information into the information system such as the service directory and the news-sheet for service users.
•Any other relevant duties according to the requirements of the Head of Service.
•Be aware of the requirements of Health and Safety Acts and work with the Directorate’s Health and Safety and Quality Assurance Officer to ensure that we comply with the relevant Health and Safety requirements.
•The Council operates a continuous programme of training and personal skills development together with an appraisal process which contributes towards this; the post-holder is expected to comply with this.
•This is an illustrative list only. The post holder will be expected to be part of the process of managing and monitoring performance in accordance with the requirements of the post and to implement any other duties in keeping with the nature of the post and its scale in accordance with the request of the Head of Service/Manager or the Strategic Director.
