We are looking to develop an additional service model, alongside the edge of care aspect of our service, to enhance our provision to meet some of the challenges described in the ‘No Wrong Door” report.
It is designed to be flexible and responsive to different levels of need, with each level providing intervention and support tailored to, and proportionate to the child or young person’s need, with a focus on providing timely help and preventing problems becoming more severe.
It’s all about families getting the right help at the right time and in a way that is right for them.
All services that have a role in mental health and wellbeing need to come together to work out how best to meet the need. These services can be from health, education, social services or the third sector. All have something to offer depending on a family’s circumstances.
The more services come together to listen to what families need, the more they can adapt how they work together to fill the gaps. Services offering extra help might focus on particular groups of children and young people, or on particular issues.
'No wrong door' helps establish what is working well in an area and what services are needed. It also prevents the frustration of waiting on a list to find it’s not the right service after all.
In a complex multi-agency network of services this is best achieved through a managed process characterised by good joint working, information sharing and mature partnerships. This post will therefore focus on a multidisciplinary approach and promoting interventions as if it were a single agency.
The Trobwynt Practice Lead would be integral to the implementation of this strategy and will be required to provide guidance, support and professional mentoring in order to influence change in culture from all partners, in order for the service to become fully integrated as well as multiagency collaboration. Trobwynt practice lead would directly manage key workers within the service to provide children with complex needs timely help and preventing problems becoming more severe. This is an excellent opportunity for an individual who has an interest in developing a new aspect to our services, with good flexible working opportunities.

Job Description
Purpose of the post
•Ensure that the people of Gwynedd are at the heart of everything we do.
•Provide active guidance and lead/change management to a workforce employed by Gwynedd Council to ensure that children and families in the area have access to the right person at the right time and receive the right support to enable them to continue to live their own lives.
•Lead on a specific work area within the team (Right Door work programme).
Responsibility for functions
•Allocate work and offer staff supervision.
•Authorise expenditure from the team budget.
Main duties
•Offer a service to children and families in accordance with a legislative framework, including allocating work and managing team members' workloads.
•Ensure that work is done in accordance with legislation, policy, guidance and accepted good practice that is relevant and lead on safeguarding issues within the team in accordance with statutory requirements and the council's policies and procedures.
•Contributing to the development of the Service and being accountable for performance monitoring (mainly responsible for the Department's response as part of developing the Right Door work programme) and contributing to the general development of the Service.
•Provide advice, guidance, support and professional mentoring to practitioners and develop professional practice within their teams and the service in general. Undertake some direct work with children, young people, families and carers and oversee the work done by the team in providing evidence based interventions and outcomes
•Ensure an effective duty rota for the service.
•Collaboration and coordination at a cross-departmental level and with other Professional agencies, independent and voluntary agencies operating in the field, including elements of commissioning work.
•Lead on a range of meetings and processes to ensure effective professional practice.
•Empowering staff to make decisions that they could defend.
• Practice in accordance with the Care Council for Wales Code of Practice for Social Care Workers
•Support and provide guidance to employees, which requires a high level of skill, assessment and analytical skills. This will include providing guidance on evidence-based policy, practice and interventions.
•Leading the Right Door programme within the team, acting as an active contact point for the development of this element of the Department's work.
•Performance and Development – Identifying staff strengths and areas of development through constant supervision and evaluation. Focus on improving performance through effective management and development and by measuring, monitoring and evaluating performance. Implement and develop business plans that reflect this. Contribute to, and develop a training programme for the team and other teams.
