•Information about accreditation schemes from external bodies (e.g. Agored Cymru, Duke of Edinburgh Award).

Job Description
Purpose of the Post.
•Ensure that the people of Gwynedd are at the heart of everything we do.
•Support Young People 16-25 identified by the Youth Engagement and Progression on a one-to-one and group basis, throughout Gwynedd.
•Plan, co-ordinate and deliver a programme of opportunities and activities for young people. The focus will be on outcomes; positive relationships; emotional, social and personal development of the young person; the promotion of good mental health and wellbeing to help young people prepare for their next steps towards education, training, employment or other opportunities.
•Establish networks so that this work can be further developed.
•Work closely with other Youth Support Teams to ensure successful transitions and good outcomes for young people.
Responsibility for functions . e.g. staff, budgets, equipment
•Supervision and organisation of projects/programmes
•Equipment used during activities with young people
•Personal equipment (ICT)
•Manage petty cash for young people – if required
Main Duties
•Offer a range of activities planned around the 5 Ways to Wellbeing model. This may include opportunities for young people to develop emotional literacy skills; nurturing healthy relationships; accommodation and independent living support, Agored Cymru accreditations, Duke of Edinburgh Award; volunteering opportunities, and projects in the Community.
•Work on an individual and group basis with young people 16-25 to develop their social, emotional, personal, and basic skills.
•Follow-up referrals received through the Youth Engagement Framework arrangements, engaging with young people to offer/receive support.
•Provide a brokerage and advocacy function for young people who wish to engage with youth support.
•Support Young People 16-25 to develop a personal action plan, achieve the goals as set out in their action plan and support to overcome obstacles (eg personal challenges, key transition period etc).
•Work closely with partners/organisations (eg third sector organisations; Gwaith Gwynedd, Youth Justice Service, Families First, Careers Wales, Grŵp Llandrillo Menai, DWP, Community Safety, and Housing Associations) and to refer/bring together provisions to support the personal development of young people.
•Provide engagement activities and outreach work to try to make contact with, and encourage young people 16-25 targeted through the Youth Engagement Framework to engage with other helpful Services.
•Prepare, organise, plan, evaluate, and manage resources for individual work programmes/activities provided.
•Promote, provide, and develop a range of learning opportunities and accreditation opportunities for young people using informal youth work methods, which are suitable for the needs of young people.
•Ensure that personal data, information, and accurate records of work are recorded within the Service’s information management system.
•Attend, contribute, and participate fully in meetings relevant to the job, the service, and the youth support field as needed.
•Comply with any procedures and systems of the Service.
•Comply with the Occupational Standards for Youth Work, the Rights of the Child.
•Prepare progress reports including monitoring and tracking young people's progress and outcomes, achievement against work programs targets/business plans/grant money as required.
•Implement and adhere to safeguarding procedures when working with young people.
•Ensure that the voice of the Young Person is central to everything that is offered and developed.
•Attend training for youth workers organised by, or on behalf of, the Council.
•Act as Site Manager at the request of the Manager.
•Responsibility for self-development.
•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
•Operate within the Council’s policies in relation to equal opportunities and equality.
•Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
