Job Description
Purpose of the Post
- Ensuring that the people of Gwynedd are central to everything we do.
- Assist the Gwynedd Actif Dementia Co-ordinator in order to develop, promote and provide key elements of the project.
- In particular, the Actif Dementia Community Officer will be responsible for contacting and understanding local communities to ensure that opportunities for physical and social activities and support are met for people affected by dementia.
- The Actif Gwynedd Dementia Community Officer will lead the work of marketing and promoting the plan's activities program.
*** “people affected by dementia”= the person with the diagnosis, family, unpaid carers and friends.

Main Duties
- Co-work with the Dementia Actif Co-ordinator in order to educate, assist and support the members who have been affected by dementia to participate in physical & social activity in their community.
- Delegating in the absence of the Dementia Actif Coordinator and making decisions on all aspects of the plan when necessary.
- Work independently and collaboratively with organizations and community groups to ensure that the needs of those specific communities are met and ensure that there are no gaps in the provision of the service.
- Being able to respond to situations and discussions and make decisions where necessary when collaborating with community groups and organisations.
- Be creative in creating and providing opportunities and a program of wide physical & social activities of high quality that are progressive and reflective of the needs of people affected by dementia in Gwynedd.
- Being responsible for promoting the programme, the programme's ideas and mission, finding new customers, and reminding existing customers of the service and support. Collaborate with the Council's Communications team to promote the activities and raise awareness.
- Be responsible for ensuring that the Dementia Actif Gwynedd website is updated and that information on social media pages is creative and up-to-date.
- Coaching classes, performing progress reviews and carrying out pre/post assessments with members. Counting other classes in the absence of other instructors.
- Start & establish a Volunteers Pack to encourage people in the community and students to participate for personal development, community spirit and to raise awareness of dementia.
- Plan and design a campaign to recruit Volunteers.
- Be proficient and confident in attending meetings with Council officers, Health Board, clinical professionals, 3rd sector organizations and community groups for the purpose of promoting the programme. And ensure effective communication with appropriate employees.
- Work closely with the plan's stakeholders, building an effective and influential relationship with officers.
- Develop opportunities and work together with the staff of Gwynedd Council's residential homes to enable residents to move.
- Raise awareness and break down barriers around dementia by providing Dementia Friends sessions for the public and local groups and assisting with the creation of Dementia Friendly Communities.
- Ensure that good practices are developed and disseminated in the delivery of exercise by researching evidence on exercise and specific medical conditions.
- Work together with Dementia Actif Co-ordinator to oversee the day to day administrative tasks of the scheme, including communicating on the telephone, distributing appropriate mail, monitoring attendance on the scheme and ensuring that members' subsequent procedures are implemented.
- To implement strict data management protocols, including confidentiality and to assist the evaluation and collection of data.
- Ensure that they understand and work within professional boundaries at all times and are responsible for their own development.
- Undertake dementia training, and anything else relevant as required.
- Other relevant duties that are set by the Dementia Actif Co-ordinator and undertake any other equivalent and reasonable duty that matches the salary level and level of responsibility of the position.
- Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities set out in the Health and Safety at Work Act 1974 and the Council's Health and Safety Policy;
