This is an exciting opportunity to be part of Gwynedd's Social Work team.
As a Social Worker you will make a huge difference to the lives of individuals here in Gwynedd by assisting them to live their lives in the way they wish to live.
You will support individuals through social and personal difficulties, at the same time as promoting their human rights and well-being. You will work with individuals, supporting their relationships and families, groups and their links with the community. You will assist them to identify their strengths, develop their skills and work on issues or challenges they may be facing.
The successful candidate will be expected to maintain their registration as a registered Social Worker with Social Care Wales and be able to demonstrate skills in accordance with the Job Description and the Person Specification (attached).
Salary: S4 / PS1 / PS2 (dependent on experience)
L1 (Newly Qualified Social Worker) - S4 (pt 26-28) £34,834-£36,648
L2 (Social Worker) – PS1 (pt 29-31), £37,336-£39,186
L3 (Lead Social Worker) – PS2 (pt 31-33), £39,186-£41,418

Job Description
Purpose of the Post.
• Ensure that the people of Gwynedd are at the heart of everything we do.
• Work as a member of the county Multi-disciplinary Mental Health Team and he/she will have responsibility for assessment, organising and providing services for people who have mental health problems and their carers. In addition, the post-holder will be expected to contact other community workers from the statutory and voluntary sectors when appropriate, to jointly organise the service to be delivered. The successful applicant will also be expected to conduct assessments under the Mental Health Act as an Approved Mental Health Professional (AMHP) or have the ability and skills to complete a AMHP course within two years of commencing the post.Responsibility for functions . e.g. staff, budgets, equipment

• To work within the service's Care and Unified Assessment Programme.
• Be an operational member of the admissions team within the service and undertake referral work and assess the needs of service users.
• Assess the needs of users and arrange and provide services.
• Evaluate, prepare and implement care plans in partnership with service users and other relevant persons.
