Job Description
Purpose of the post
The purpose of the Integrated Adults Team is:
“to enable me to live my life as I wish”.
•Assist vulnerable individuals to live their lives in the way they wish to live
•Ensure that the people of Gwynedd are at the heart of everything we do.
•Assess the needs of the adults and their carers in accordance with the requirements of the Social Services and Well-being Act 2014 and other relevant legislation and guidelines.
•Provide an Information, Advice and Support service to citizens to enable them to meet their personal objectives main duties
•Assist people to identify solutions, prevent dependency and promote independence
•Provide information about the support resources available in the community, from the third sector, the Council and the Health Board, to enable citizens to live as independently as possible and to maximise their quality of life.
•Ensure that individuals' needs are met in accordance with the Council's safeguarding arrangements and statutory responsibilities.
Responsibility for functions
Staff - No direct management responsibilities but is expected to work as part of a team by supporting and mentoring co-workers and students in training. Give professional support to Social Work Practitioners when appropriate.
Finance - Provide advice and practical and appropriate guidance to service users in order to effectively facilitate the Service's financial management.
Equipment – Responsibility for all specialist equipment used to carry out the job. Demonstrate the best use of resources in regards to the provision of adaptations and appropriate equipment and provide a service in accordance with policies, procedures and guidelines. Responsibility for the keys to the storerooms where the equipment is kept.
Main duties
SPECIFIC RESPONSIBILITIES
•Listen to people, talk about what's important to the citizen by asking about their views about their strengths, needs, risks and circumstances and respect those viewpoints.
•Promote the rights and interests of individuals, family members, carers, partners and other people who are close or important to the individual.
•Promote and ensure that the citizen's voice is heard and that their personal situation is considered and that a discussion takes place about their personal well-being objectives. Support individuals to get advocacy support.
•Ensure that what is important to the citizen is central to everything and that every response is proportionate and tailored to the citizen with the response being agreed upon in full partnership with the citizen.
•Assist the citizen to implement options/and reach well-being objectives according to what is important to them by weighing up a range of possible answers and consider which resources the citizens themselves or family members, friends or the local community have that could be of assistance.
•Recognise that personal objectives are not only achieved by providing services.
•Focus on discovering creative ways of fulfilling citizen's objectives without restricting the intervention to match people with traditional or current services and creating a dependency on services.
•Recognise that the citizen is the expert on their life and assist them to note what would help them and build on their strengths and abilities by supporting them to make informed decisions.
•Analyse the nature, urgency level and implications of any risks noted and respond appropriately and in a timely manner.
•Assess the citizen's mental capability to make decisions by making the best decisions for the citizen's well-being if they are assessed as not having a mental capacity in accordance with the Mental Capacity Act.
•Assess the individual's needs and requirements from the perspective of minor adaptations, main adaptations and complex structural adaptations (including extensions). Consider the need for suitable accommodation. Ensue that the adaptations are completed to an acceptable standard which meets the requirement.
•Asses, plan, provide and display the use of specialist techniques or equipment to individuals, carers and care providers.
•Complete a care and support plan for an individual or carer where there is a responsibility on the Local Authority to meet their needs in accordance with the eligibility criteria for a Care and Support Plan in accordance with the Act.
•Review the care and support plan to ensure that the care provision meets the individual's needs. Ensure that equipment is returned when no longer needed. Appropriately use the equipment lending service.
•Maintain and record comprehensive manual handling risk assessments in accordance with the Act and guidelines. Develop, plan and review manual handing plans in respect of others to manage risk to the individual and carers.
•Be aware of the steps that should be taken in cases where there are allegations of harm or abuse in accordance with section 7 of the Social Services and Well-being Act. Complete investigations into allegations of abuse.
•Work as part of a multi-disciplinary integrated team and respect the skills, expertise and contributions of colleagues within the Team. Take joint-ownership for working in an integrated manner.
•Retain ownership of cases. Undertake responsibilities of a care coordinator by 'bringing in' expertise as and when needed.
•Collaborate with every Department within the Council and other partners who have a contribution to make to ensure that citizens meet their well-being objectives.
•Establish and maintain links with community resources. Research and strengthen information about community resources that exist locally.
•Communicate in an open and informed manner by adapting to different audiences and present professional decisions verbally and in writing.
Be accountable for the quality of the work and take responsibility for maintaining and developing information and skills.
•Be responsible for self development in accordance with the requirements of HPC/COT. A continuous professional development portfolio must be kept. Actively participate in regular personal and group supervision sessions and in the annual evaluation process.
•Opportunities to complete Best Interest Assessor and Adult Mental Health Practitioner (Mental Health Act) training in accordance with the service's needs and the Department's resources.
•Record accurately, proportionally and timely to ensure that our integrated records systems are up-to-date.
•Be open to propose and implement ideas and new ways of working.
•Work in a preventative way to reduce dependency on services.
•Develop and maintain an effective working relationship of high standard based on trust and respect, with a wide variety of co-workers and external agencies, including third sector and voluntary organisations, care providers and communities.
•Explain the Council's charging policy for services to all service users. Provide direct support and/or act as a presenter to the Income and Welfare Unit to ensure that every service user receives support to apply for financial support from the Council to pay for their services.
•Ensure that a standard agreement is completed between the Council and users for every service provided.
•Discover solutions to overcome barriers. Report on obstacles that cannot be overcome locally to the leader.
•Be aware and contribute to shaping the commissioning procedure for adults social care as needed. This includes mapping community services and contributing to the Population Needs Assessment.
•Achieve desirable results for service users. Conduct an effective procurement of service whilst minimising costs to the user and the Council. Maintain effective procurement of services by minimising costs to the user and the Council.
•Contribute to developing and achieving well-being objectives within the Council.
•Make full use of information technology to facilitate effective and efficient service provision.
•Ensure compliance with any risk assessment arrangements identified in risk assessments that are relevant to the work.
•Commitment to providing high-level customer care and assist people in accordance with the principles of the Act.
•Deputise for the Senior Practitioner or Area Manager as required.
•Expectation to occasionally work beyond the boundaries of the work area.
OTHER RESPONSIBILITIES
•Responsibility for self-development.
•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
•Operate within the Council’s policies in relation to equal opportunities and equality.
•Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.

Special circumstances
•Working unsociable hours occasionally as and when necessary.
•Consideration is being given to extending the service's opening hours during the week along with working weekends.
•The post-holder will be expected to be part of any arrangements that will be established on a rota basis.
