MEDICAL REGISTRATION FORM completed by Dr. Henry Thomas, 10 White Friars, Chester requesting his registration as a medical practitioner. Printed: Endorsed: LETTER (1 Feb. 1859): J.M.Davison to Dr. Thomas stating that he had presented this form with £2.11.0d but it could not be accepted by the Medical Council. Dr. Thomas must send his diploma or copy of it to the Medical Council. Mr. Davison concludes by stating that he does not think that Dr. Thomas need have any fears as according to the Act they are bound to register him.
